Diagnosis Code

This article will explain how to access List Maintenance and add a new diagnosis code. List Maintenance is a user permission, if the current user doesn't have this permission they will not be able to add a new diagnosis code.

Dashboard

First, Navigate to List Maintenance

If you have the correct permission, you can click on the List Maintenance menu item from the dashboard.

Next, click on the Diagnosis tab.

Diagnosis = the name that will show in the system. You can add the code in the name if that is something you will want to see.

DSM4 Code = (optional) The DSM4 code for the diagnosis
DSM5 Code = (optional) The DSM5 code for the diagnosis

ICD9 Code =  The ICD9 code for the diagnosis (http://www.icd9data.com/). This value does not need to be entered if you are not billing services prior to 10/1/2015 with the diagnosis.

ICD10 Code = (required) If you plan on billing for services with this diagnosis, the ICD10 code is what is used when the CMS1500 or EDI is generated

When you first login to Unicentric click on the Tools tab, running down the left hand side of the screen will be all the system tools you have access to. Assuming you have permission to List Maintenance, click on it.

After selecting List Maintenance there will be a series of list you can choose to edit, they're shown below the Tool tab. Please select "Diagnosis" from this set of options, it should show a new sections with the title of "Diagnosis Maintenance"(pictured above). In this new section you will see options below the header: Axis I, Axis II and Axis III, if your system is still using multiple axises then select the one you wish to add the diagnosis to, default is Axis I. Next select the "Add" button, this will show yet another section(pictured above) where you can enter information for the new diagnosis. Once all the necessary information has been entered select the "Save" button.

*NOTE* If claims with dates of services before 10/1/2015 will be billed with this diagnosis then an ICD9 Code will be required, otherwise only an ICD10 Code is necessary. For ICD10/ICD9 conversions please use this site: http://www.icd10data.com/

If everything was done correctly your new diagnosis code should show in the list box and can be edited or deleted form this point.
